Texas attorney general investigates UnitedHealth coverage practices

The Texas attorney general announced an investigation and Civil Investigative Demands concerning alleged deceptive and unlawful coverage practices. The release describes allegations involving nursing-home hospitalization incentives and authorization withdrawn after a procedure.

Why it matters: These are allegations under investigation, not adjudicated findings, a settlement or a fine. The company response was not independently retrieved. The release does not establish denial-related deaths or a quantified financial recovery.

Washington reports a $5,000 UnitedHealthcare fine over clinical review

Washington OIC reported a $5,000 fine against UnitedHealthcare of Washington, Inc., order 26-0027. It describes 50 adverse benefit determinations or access limits for gender-affirming care without review by a clinician experienced in that treatment.

Why it matters: October 5 is the publication date, not a verified order date; the release covers April-August 2026. Payment, insurance program and remediation are unconfirmed. The $495,200 release-wide total is not UnitedHealthcare liability. Indexed primary release retrieved; direct page returned 403 and underlying order remains pending.

UnitedHealthcare emphasizes HMO coverage in its 2027 portfolio

UnitedHealthcare announced a 2027 portfolio emphasizing HMO plans and specialized plans for people with chronic conditions or both Medicare and Medicaid. It says approximately 95% of Medicare-eligible people within its footprint will have an HMO option.

Why it matters: The percentage describes people in the company footprint, not the entire country or enrolled membership. Members need to verify whether their clinicians, hospitals and medications remain covered.

What the documented record shows.

Appeals must lead to action

The California record documents delays after independent reviews overturned denials. It raises a specific operational question: how quickly are approved treatments and provider claims completed? Primary source

Different businesses, different obligations

OptumRx records concern PBM and pharmacy operations. Combining those amounts with Medicaid health-plan fines would obscure which business and program were involved.

Compensation needs context

Hemsley’s reported 2025 compensation includes a roughly $60 million option award intended to cover multiple years. The accounting value is not cash received in 2025. Primary source

Fines, settlements and audit findings.

Amounts retain their original category and payment status. There is no combined “fines paid” total.

Settlement announced; consumer distributions subsequently reported

Mental health coverage and reimbursement practices challenged under parity requirements.

NY reported $13.6 million distributed in 2022 plus $725,000 paid in 2021. This is one action, not two settlements. Follow-up: https://ag.ny.gov/press-release/2022/attorney-general-james-provides-136-million-consumers-who-were-denied-mental

Sources, methods and limits

A collection readers can check.

Historical base: January 2020–October 6, 2026. Targeted October 12 update adds Washington order 26-0027 and the unresolved Texas investigation in recent news. This is not a complete current review. Earlier conduct is included when an action was announced during the window. Sources prioritize regulators, state procurement notices, SEC filings and company disclosures. Company award announcements are identified as such.

Subsidiaries are grouped under their parent, with the entity and business identified. Former subsidiaries are labeled. A source link appears with each record. Reporting an allegation is not a finding of liability.

  • Initial collection, not a complete inventory of every action, subsidiary or procurement from 2020–2026.
  • Payment confirmation and appeal dispositions remain incomplete; assessed or agreed amounts are not described as paid.
  • Earlier executive compensation extraction is incomplete: 2020 all companies; 2021 CVS; 2022 UnitedHealth. Earlier non-CEO pay and benefits remain to be extracted.
  • 2026 annual executive compensation is not yet reported for a completed fiscal year. Positions in the 2025 pay table reflect that disclosure, not a current leadership directory.
  • CVS detailed benefit breakdown remains to be extracted. All other compensation is not synonymous with benefits; it can include severance and other items.
  • No matched evidence establishes elimination and later reposting of the same positions. Need job title, requisition, duties, location, employing entity and dated notices/postings.
  • Initial contract collection is selective. No Humana contract loss is documented here; that is a coverage gap, not proof that none occurred.
  • Pharmacy, PBM, employer insurance and workers compensation actions are labeled separately from Medicaid / Medicare health-plan actions.
  • Unequal search coverage and company size prevent a defensible ranking of companies by the counts or amounts in this collection.

Analysis expresses questions and inferences from the listed records. It does not establish causation between executive pay, staffing changes and member outcomes.
